Complaint Investigation Summary Report#: 5105 <br /> COMPLAINT ID: CO0047134 Site Location: 1000 ROTH RD <br /> Activity Summary <br /> Activity Date Recorded by <br /> 07/22/2018 8:30 am-9:12 am: UP spill MANZO <br /> 8:30 am: Received a page from General Hospital concerning 5 gallons spill of <br /> unknown liquid. Received spill information. Called Christen with UP 888-877-7267, <br /> she provided a report number 2018-07-22-083 KLV. Christen reports a container <br /> which on paper was only supposed to carry dry goods is leaking an unknown <br /> liquid. Container number is UMXU251059. Called Mike Algots 916-580-9999, he <br /> reported that Patriot is in route and XPO/Amazon appears to be the shipper. He ill <br /> provide more information once they open the container. <br /> 11am called Mike Algots, they opened the container and it's an amazon trailer with <br /> lots of loose small boxes. Patriot located a small leak on the underside of the <br /> container. Since they need a large container to be provided by the shipper to <br /> unload all the loose items, their plan is to wrap the underside of the container to <br /> stop the leak and dike around the container and unload tomorrow when the <br /> shipper provides a second container. No hazardous materials were listed on the <br /> shipping papers. <br /> 11:26 am, received a call from Jason Morgan with Patriot 916-936-7833, the liquid <br /> appears oily no apparent odors and pH is at 6. He will provide an update onse they <br /> secured the trailer with absorbent pads. <br /> Reviewed an email received this morning from Michael J. Algots <br /> <mjalgots@up.com>: `UMXU 251059 was reported to have a liquid leak from the <br /> bottom. Estimated at 5 gallons to the ground. The container is billed as non-haz. <br /> The EDI information shows Amazon so it may be mixed goods. Patriot will <br /> investigate, clean, and secure the release and also confirm that there is no hazmat <br /> in the container. The shipper will need to hire a company to rework/block and <br /> brace the container prior to being released. Waste generated will be the <br /> responsibility of the Shipper(XPO Intermodal) along with response costs. Seth, <br /> please contact the shipper and advise them on the state of their container and <br /> their responsibilities, please provide Patriot and SJCEHD with a contact for direct <br /> billing. Patriot, Please provide updates through this email chain." <br /> Reviewed an email from M. Algots from 11 am: "All, the container is full of <br /> boxes/packages(no pallets).We will need another container to offload into, as the <br /> leak is coming from the nose of the container. The leak has been described as a <br /> oil like liquid. Patriot will apply secondary containment under the container to <br /> secure the leak until we have the second container. Shipment Quality/Lathrop, will <br /> you please get a container to trans load into?" <br /> 4 pm, received pictures from Jason Morgan showing how the trailer was secured <br /> pending clean up. <br /> 07/23/2018 Called Mike Algots with Union Pacific Railroad (UPRR) as requested by lead RUVALCABA <br /> senior. Lead senior received spill notification on the morning of 07/22/2018. Lead <br /> senior stated that UPRR had yet to perform cleanup. Mike stated that responsible <br /> party (RP) could not be contacted. The shipping container had to be emptied and <br /> the RP needed to supply another container. They were working on getting in <br /> contact with the RP. Mike stated that he was in contact with lead senior through <br /> email and would keep her updated of the situation. <br /> 5105 rpt Page 2 of 3 <br />
