Complaint Investigation Summary Report#:5105 <br /> COMPLAINT ID: CO8S2HYYN Site Location: NEAR 2373 S. MARIPOSA RD. <br /> Activity Summary <br /> Activity Date Recorded by <br /> 08/13/2020 5pm GAITAN <br /> This is a continuation from a day time complaint. <br /> Met with Juan Armendarez, District Hazmat manager for Caltrans. (209)483-3088 <br /> He showed me where the storm drain was impacted. Storm drain, located directly <br /> in front of Maxim Crane Works was the initial impact site. Juan stated that the <br /> storm drain leads north, across the street, and veers west towards Frontage Rd. <br /> Juan stated that an employee from City of Stockton storm drain unit was on site <br /> earlier and confirmed that the storm drain west of Frontage Rd. was dry. Juan <br /> forward me a copy of the responsible party contact information. Juan stated that <br /> the storm drain is part of SJC property. Juan Stated that a mechanic for the <br /> responsible party arrived on site and cleaned-up the diesel on the asphalt and took <br /> the used absorbent with him. He stated that he was told a nearby fire department <br /> was on site and left once the diesel on the asphalt was cleaned up. <br /> Contacted Stacy Rivera 11-ead Senior REHSj and discussed the complaint. After <br /> discussion Stacy stated that she will contact Public Works to find out if they would <br /> perform the clean-up. I contacted the responsible party to see if they will have a <br /> hazmat materials clean-up contractor perform clean-up. Contacted the truck <br /> driver, Harmanprit Singh, (541) 809-9430 Asked if any additional clean-up was <br /> going to be performed on storm drain impacted with diesel. He stated that he does <br /> know, he gave me the contact information for his boss, Dwinder Singh, (209) <br /> 623-3570. Contacted Dwinder and asked if a clean-up contractor will be cleaning <br /> the storm drain impacted with diesel. He stated that the fire department and City of <br /> Stockton told him that everything was already cleaned-up. I informed him that the <br /> storm drain still required cleaning. He stated that I should call his insurance if I <br /> want the storm drain to be cleaned up. I informed him that he needs to call his <br /> insurance since he is the responsible party. I also informed him that if he does not <br /> hire a clean-up contractor then the county may perform clean-up at a higher rate. <br /> He stated that is okay by him. <br /> Contacted Stacy and discussed the conversation I had with Dwinder Singh. Stacy <br /> gave me the contact information for Wyatt McCallister, on-call employee for public <br /> works. (209) 468-3074. Stacy stated that Wyatt confirmed that he does not have <br /> the equipment to clean-up the storm drain. After discussion I contacted Dwinder <br /> Singh, asked if he was going to contact his insurance and pointed out that this a <br /> serious matter that required immediate attention. He stated that he will have his <br /> insurance contact me, he requested I send him an email, that he can forward to <br /> his insurance. Generated a notice to abate and sent to his email <br /> BIS95212@gmail.com. Dwinder stated that he will notify his insurance company <br /> and have them call me. Contacted Wyatt McCallister and discussed options to <br /> clean-up the storm drain, notified him that the responsible party does not have a <br /> clean-up contractor at this time. He stated that he might have a clean-up <br /> contractor that can perform the work, he stated that he will call around. <br /> Met with Wyatt, he opened up the storm drain grate. I observed staining going <br /> down the side of the wall leading into the storm drain. Could not observe any <br /> sheen. He stated that public works, does not have any contractor that can <br /> clean-up the storm drain. Gave him a copy of the responsible party contact <br /> information. Contacted Stacy, based on discussion I left site and waited on a <br /> response from the responsible party. <br /> 08/13/2020 Received a call from Juan Armendarez, District Hazmat Manager for Caltrans. GAITAN <br /> (209) 483-3088 Stated that he received a notification of a semi-truck that leaked <br /> approximately 20-gallons of diesel on to asphalt and a nearby storm drain located <br /> in front of 2373 Mariposa Rd. Stockton, CA 95205. Stated that I was still in Tracy <br /> for the 1-205 spill. Contacted Garrett Backus ILead Senior REHS1 and discussed <br /> this complaint. Based on discussion I left Tracy and headed over to Stockton. Sent <br /> Juan a message that my ETA will be 5pm. <br /> 5105.rpt Page 2 of 6 <br />
