She needed assistance understanding the SINGH <br /> forms. <br /> Received an IPCP from Cassi via email. <br /> 04/02/2021 Received a couple of text messages in regards to the Service Request form from SINGH <br /> Cassi. I emailed Cassi another Service Request form. <br /> Reviewed Infection Prevention and Control Plan. <br /> 04/07/2021 Received a text message from Cassi asking for the Service Request Form again SINGH <br /> and the online payment link. I emailed both items to Cassi. <br /> 04/12/2021 Received facility layout via email from Cassi. Responded to a few SINGH <br /> correspondences and received a call from Cassi for assistance with online <br /> payment. Emailed her another Service Request Form. <br /> 04/13/2021 Received completed Service Request Form from Cassi. SINGH <br /> 04/14/2021 Dropped off Service Request to Accounting at the main building. SINGH <br /> 04/20/2021 1 contacted Cassi to schedule the consultation. Cassi will look at her schedule for SINGH <br /> next week and then get back to me. <br /> 04/30/2021 The consultation is scheduled for 5/6/21 at 9 am. SINGH <br /> 05/04/2021 Reviewed file. Prepared for the consultation. SINGH <br /> 05/19/2021 Updated and reviewed file. Abated complaint to field response-violations cited SINGH <br /> and corrected.
