One hand-wash sink per two practitioners. <br /> 2. Hand-wash sink must be permanently fixed and able to supply hot and cold running water. <br /> 3. Hand-wash sinks in the procedure room and restroom must have containerized liquid soap and mounted touchless <br /> paper towel dispenser. <br /> 4. The wall must be free of holes, smooth and easily cleanable. <br /> 5. Floor must be non-porous and easily cleanable. <br /> 6. Sterilization certificates and invoices for all purchased single-use needles/blades must be available for review by <br /> the EHD. <br /> 7. A receipt or contract from a licensed waste hauler/mail-back company for the sharp container must be available <br /> for review by the EHD.The sharps container must be within arm's reach. <br /> 8. Client consent form, medical questionnaire, and after-care instructions must contain all the requirements in Form <br /> B (attached). <br /> 9. Infection Prevention and Control Plan must be complete and available for the EHD to review. An Infection <br /> Prevention and Control Plan template is attached, you can either use the provided template or any other form of your <br /> choice. <br /> 10. Explain how you set-up and break-down during the procedure. <br /> 11. All instruments and tools used on the clients must be single-use. Please have instruments/tools available for <br /> inspection. <br /> 12. Your bloodborne pathogen training certificate must be current and posted. Attached is a list of approved training <br /> providers. <br /> After the consultation, an inspection report will be provided to you.The inspection report will consist of a list of <br /> corrections that will be required to be completed prior to being permitted to open the shop. <br /> Please be advised that there is a facility operating fee of$233 (disposable instruments used) or$304 (sterilization used) <br /> and a $152 body art practitioner permit fee, prior to opening the facility. <br /> You can find COVID-19 guidelines by clicking on the following link: <br /> https://www.sigov.org/department/envhealth/programs/default?id=27360). <br /> Let me know if you have any questions. <br /> 1 <br />
