Subject: PR0520991; CHUCK E CHEESE'S#631 Hazardous Materials Business Plan Inspection; Employee Training Records <br /> Request <br /> Ricard Santos, <br /> As a follow-up from today's phone call, I advised the EHD will be conducting the Hazardous Materials Program <br /> Inspection via phone call; prior to the inspection I am requesting a copy of the employee training records for the past <br /> three years showing the following required business plan training elements have been addressed: <br /> • Business plans shall include provisions for ensuring that appropriate personnel receive initial and annual <br /> refresher training. All employees shall be trained in safety procedures in the event of a release or <br /> threatened release of a hazardous material, including, but not limited to,familiarity with the following <br /> plans and procedures: <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the <br /> unified program agency. <br /> (B) Procedures for the mitigation of a release or threatened release to minimize any potential harm or <br /> damage to persons, property, or the environment. <br /> (C) Evacuation plans and procedures, including immediate notice,for the business site. <br /> This training shall be documented electronically or by hard copy and shall be made available for a <br /> minimum of three years. <br /> Please,forward the employee training records to me on or before July 28, 2021. <br /> As of September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
