Environmental Health Department <br />Hazardous Materials Program Inspection Report <br />Date: <br />July 22, 2021 <br /> Facility Address: <br /> 710 E YOSEMITE AVE, MANTECA <br /> Facility Name: <br /> ON THE SPOT AUTO <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br />OBSERVATION: The site map was not completed and submitted as part of the business plan. The site map section <br />had a comment saying the site map is provided else where in CERS. The site map was not submitted for the years <br />of 2019, 2020, and 2021. The site map was last submitted on 3/2/2018. <br />REGULATION GUIDANCE: A site map shall contain a north orientation, loading areas, internal roads, adjacent <br />streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br />material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as “NA". <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This violation was corrected on site. <br />This is a repeat violation, Class II. <br /> 7 HSC 25505(a)(3) Failed to electronically submit an adequate ER procedures for a release or threatened release. <br />OBSERVATION: The emergency response(ER) procedures were not submitted. A comment was left in the <br />emergency response and contingency plan section saying provided else where in CERS. The emergency response <br />and contingency plan was not submitted for the tears of 2019, 2020, and 2021. The last submittal made was on <br />3/2/2018. <br />REGULATION GUIDANCE: A business plan shall include the following emergency response procedures for a <br />release or threatened release of hazardous materials, including, but not limited to, the following: <br /> - immediate notification of local emergency personnel and the Environmental Health Department (EHD) <br /> - procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br />persons, property, or the environment <br /> - evacuation plans and procedures, including immediate notice, for the site <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This violation was corrected on site. <br />This is a repeat violation, Class II. <br />FA0012457 PR0520822 SC001 07/22/2021 <br />EHD 19-01 Rev. 9/16/2020 Hazardous Materials Program OIRPage 3 of 5 <br />1868 E.
