The following is an itemized list of hazardous materials violations that have not been addressed for CALIFORNIA HUMAN DEVELOPMENT CORP as of September29 , 2021 . Open violations from August 17 , 2021 inspection Violation #6 - Failed to electronically submit a site map with all required content . 1 . OBSERVATION The site map has not been annually certified/submitted as a part of the business plan . The site map was not submitted for 2019 , 2020 , and 2021 . The site map has not been submitted since March 22 , 2018 . REGULATION GUIDANCE: Per H & SC 25508 . 2 , the business owner/operator shall annually review and certify that the business plan is complete , accurate , up-to-date and in compliance with Section 11022 of Title 42 of the United States Code . An annual electronic submittal satisfies these requirements . The annual business plan shall be submitted/certified by January 15 of each calendar year and may be submitted beginning November 1 of the previous year. One of the maps does not display a north orientation . 3 . Loading areas . 4 . Internal roads . 5 . Access and exit points from the street . 6 . Emergency shutoffs (s) . 7 . The actual location of the oxygen and acetylene and the location of the disinfectant/fungicide and latex paint . REGULATION GUIDANCE: A site map shall contain a north orientation , loading areas , internal roads , adjacent streets , storm and sewer drains , access and exit points , emergency shutoffs , evacuation staging areas , hazardous material handling and storage areas , and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and label as " NA" .
