The following is an itemized list of hazardous materials violations that have not been <br /> addressed for CALIFORNIA HUMAN DEVELOPMENT CORP as of September29 , <br /> 2021 . <br /> Open violations from August 17 , 2021 inspection <br /> Violation #6 - Failed to electronically submit a site map with all required content . <br /> 1 . OBSERVATION The site map has not been annually certified/submitted as a part of the business plan . The site <br /> map was not submitted for 2019 , 2020 , and 2021 . The site map has not been submitted since March 22 , 2018 . <br /> REGULATION GUIDANCE: Per H & SC 25508 . 2 , the business owner/operator shall annually review and certify that <br /> the business plan is complete , accurate , up-to-date and in compliance with Section 11022 of Title 42 of the United <br /> States Code . An annual electronic submittal satisfies these requirements . The annual business plan shall be <br /> submitted/certified by January 15 of each calendar year and may be submitted beginning November 1 of the <br /> previous year. <br /> CORRECTIVE ACTION : Immediately log into the CERS at http : //cers. calepa . ca . gov/, review and submit/certify the <br /> current information . <br /> . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . <br /> 2 . Access and exit points from the street . <br /> 6 . Emergency shutoffs (s) . <br /> 7 . The actual location of the oxygen and acetylene and the location of the disinfectant/fungicide and latex paint . <br /> REGULATION GUIDANCE: A site map shall contain a north orientation , loading areas , internal roads , adjacent <br /> streets , storm and sewer drains , access and exit points , emergency shutoffs , evacuation staging areas , hazardous <br /> material handling and storage areas , and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as " NA" . <br /> CORRECTIVE ACTION : Immediately log into the California Environmental Reporting System ( CERS ) at <br /> http : //cers, calepa . ca . gov/ , upload the correct or updated information , and submit to the EHD for approval . <br /> ❑ This violation was corrected ❑ This violation will be corrected by ( date) : <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation : <br /> Page 1 of 2 <br />
