The primary <br /> contacts 24-hour phone number of 209-597-1525 was not correct. Phone number does not connect to PG&E <br /> employees or PG&E dispatch center. <br /> REGULATION GUIDANCE: This information must be current and submitted as part of the business plan. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 9 HSC 25508.2 Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br /> OBSERVATION: The business plan information has not been reviewed and resubmitted in the California <br /> Environmental Reporting System (CERS)annually by the due date, January 15. The facility, inventory, and plans <br /> section of the Hazardous Material Business Plan (HMBP)was last submitted on 1/15/2020.The facility, inventory, <br /> and plans sections of the HMBP have not been submitted for the year of 2021. <br /> REGULATION GUIDANCE: The hazardous materials inventory shall be submitted by January 15 of each calendar <br /> year and may be submitted beginning November 1 of the previous year. <br /> CORRECTIVE ACTION: Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated <br /> information, and submit to the EHD for approval. <br /> This violation has been corrected. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> An inspection report was provided to the facility operator on the day of inspection. The report was amended on <br /> 10/4/2021 and replaces the initial report on 8/11/2021 and the amended inspection report issued on 8/13/2021. <br /> The inspection report completed on 8-11-2021 said no violations were found at time of inspection. Further <br /> review has determined the Hazardous Material Business Plan (HMBP) has not been submitted for the year of <br /> 2021. This is why violations#9 has been added to the inspection report. violation#4 has been issued because <br /> the primary contact phone number is not correct. More details are provided in the comment section for each <br /> violation. Violation#5- 8 were rescinded. Violations#4 and #9 have been changed from class II violations to <br /> minor violations. The RTC Qualifiers say documented and cannot be removed. Violation#9 say corrected on <br /> site.
