S A N J 0 A Q I I I Irl Environmental Health Department <br /> COUNT <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Pilot Travel Center Lathrop- 1017 345 Roth Rd, FRENCH CAMP August 18, 2021 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 319 CCR 2635, 2637.2,2665, 2712 Failure to comply with overfill prevention equipment requirements. <br /> OBSERVATION: The mechanical flapper used as the overfill prevention system is not currently functional on the 87 <br /> UST. <br /> REGULATION GUIDANCE:All UST systems must be equipped with an approved,functional overfill prevention <br /> device at all times during delivery operations. <br /> CORRECTIVE ACTION: Immediately discontinue deposition of petroleum into this tank until the component is <br /> repaired or replaced by a properly licensed, trained, and certified contractor under permit and inspection of the EHD. <br /> If the failed component can't be replaced immediately,there is a possibility that the 87 UST system may be red <br /> tagged to prevent fuel inputs. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by September 17, 2021. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152).
