5208 S.Lawton <br /> ' Tulsa OK, 07 <br /> 918-998-086 <br /> Date: 12/15/2016 <br /> A C U R E ly Location: Owens Brockway Tray CA <br /> Equip.#: Emergency Generator A <br /> Cert.Level: API-653#36530 <br /> Inspector: Gregory Fann <br /> Examination Report <br /> Scope: <br /> 1)External visual inspection. <br /> 2)Ultrasonic thickness examination of shell,roof and floor. <br /> Summary: <br /> The subject 140 gallon capacity atmospheric storage tank was inspected per the above scope and evaluated per the specified STI SP-001. <br /> Inspection of the tank bottom was not possible due to proximity of the floor.Further details may be referenced in the attached Checklist,Calculation sheet, <br /> TML report,Image sheet,or Drawing. <br /> Results: <br /> 1)Oil contamination from generator engine. <br /> 2)Insufficient containment. <br /> 3)No level gauge. <br /> Recommendations: <br /> 1)Remove contaminant from tank,repair leaking component.(do not leave absorbs on tank,they will accelerate corrosion) <br /> 2)Add adequate containment around tank to reduce consequence of failure. <br /> 3)Add level gauge or other means of determining fuel level. <br /> 4)Continue scheduled inspection program per STI SP-001 and O-I company policies and procedures. <br />
