T the two 55 gallon drums of Amidex CE Surfactant have not been reported. <br /> 2. The 265 gallon tote of Neutralac SLS 45% has not been reported. <br /> 3. The four 55 gallon drums of AOS-40 have not been reported. <br /> 4. The two 55 gallon drums of Glycolic Acid 70% have not been reported. <br /> 5. The 55 gallon drum of degreaser has not been reported. <br /> 6. The 55 gallon drum of ECO9000 has not been reported. <br /> 7. Iron Chloride—the days on site element has not been completed. <br /> 8. Hydroxyacetic acid—the chemical location element has not been completed. <br /> 9. Hydrochloric acid—the chemical location element has not been completed. <br /> • Violation #6,the site map does not show or address the following: <br /> 1. Adjacent street. <br /> 2. Access and exit point, including gate,from the street. <br /> 3. The locations of the Amidex CE Surfactant, Aquamet M (sod iudiemethyldithiocarbamate), Neutrala CSLS <br /> 45%, AOS-40, glycolic acid 70%, glacial acetic acid, ferric chloride solution, degreaser,ferrous sulfate, <br /> ECO1200 (polyaluminum Hydroxchlorosulfate blend), potassium hydroxide, ECO9000, hydrated lime, <br /> cocamides diethanolamines, hydroxyacetic acid, and hydrochloric acid. <br /> A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br /> access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, <br /> and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and <br /> label as "NA". <br /> • Violation #11—Records showing the employees have received training in the following elements have not been <br /> received (provide completed employee training record directly to the EHD for review). <br /> 1. Familiarity with the emergency response plans and procedures in the event of a release or threatened <br /> release of a hazardous material; (Emergency Response/Contingency Plan familiarization). <br /> 2. Immediate notification contacts to the appropriate local emergency response personnel and to the <br /> unified program agency; (spill reporting). <br /> 3. Procedures for the mitigation of a release or threatened release to minimize any potential harm or <br /> damage to persons, property, or the environment; (spill response). <br /> 4. Evacuation plans and procedures, including immediate notice, for the business site; (evacuation <br /> training). <br />
