Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: C051XRFTL Site Location: 1000 E ROTH RD <br />Activity Summary <br />Activity Date Recorded by <br />05/18/2020 Received email from Michelle Sims, (Celtic). She reported that Celtic is working MURO <br />diligently to get a copy of the disposal record. The issue is that Patriot <br />Environmental Services who performed the cleanup, has been unresponsive to <br />their requests for documentation. The primary contact for Patriot they have, Joe <br />Mcguffin, was copied in the email. He was the Project Manager with Patriot <br />Environmental Services for the incident. <br />06/04/2020 Received email from Laurie Brown (EHD) requesting confirmation that the <br />responsible party should be changed to Celtic International. I discussed with Elena <br />Manzo, lead senior. She confirmed that the RP should be updated. <br />Provided mailing address to Laurie Brown via email. <br />Updated RP in ECR and synchronized EC. <br />08/05/2020 Sent email to Michelle Sims (Celtic International) to ask if they managed to <br />procure a copy of the disposal record from Patriot Environmental. <br />Received reply from Michelle Sims that Patriot was unable to reproduce <br />documents of destruction. <br />08/05/2020 Reviewed past emails and attachments. Printed emails. <br />Consulted Lead Senior Elena Manzo regarding Celtic Intermodars inability to <br />obtain disposal records. Michelle Sims reported that Patriot cannot reproduce any <br />documents of destruction. <br />10/20/2020 Received a call forward from Michaela Zimmerman. Caller was Sharon Jordan <br />(Celtic International) who had questions for invoice charges for 05-27-2020, but <br />had no complaint number. She wanted to know what the charges were from so <br />she can link load and trailer information for payment. I found the complaint and <br />explained the details. I let her know that the charges most recently were from the <br />time I spent requesting disposal records so that I may close the complaint. I let her <br />know that Michelle Sims (Celtic Intermodal) was not able to provide EHD with <br />disposal Records. I asked if she can look into it and provide disposal records, I <br />can close the complaint. I forwarded the thread to her at sjordan@celticintl.com <br />so that she has background information. <br />I received a reply from Sharon stating she was unable to obtain documentation for <br />the disposal, and if I can close the complaint based on age. I replied that I will put <br />the complaint file together and submit for review and contact her if we will need <br />anything further. <br />12/22/2020 Printed emails, 4 SDS sheets (Power Stick 3-in-1 Body Wash, Arm & Hammer <br />Essentials Body Wash, Arm & Hammer Ultra Max Body Wash, Power Stick <br />Shower Gel + Shampoo), report #s: 5104 and 5105, and updated log. Made a few <br />edits. Compiled confidential and non-confidential packets. Scanned both packets <br />then emailed to Lead Senior Elena Manzo (EHD) for review.
