Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: C00051835 Site Location: NB 1-5 AT MATHEWS RD OFF RAMP <br />Activity Summary <br />Activity Date Recorded by <br />06/10/2021 Printed documentation, organized complaint, reviewed Corrective Action Report MANZO <br />received from ERTS, dated June 2, 2020. Two 20 yard bins of contaminated <br />material was disposed of on non-hazardous waste manifests. Two sampled were <br />collected from the yard bins for waste profiling, thee samples were collected from <br />the bottom of the excavation. The two samples collected from waste profiling had <br />no metal concentrations exceeding the Title 22 concentrations set for hazardous <br />waste determination (with the exception of lead). Lead was detected at 73.5 mg/kg <br />(Bin #20244 sample) and at 104 mg/kg (Bin #2). No STLC results could be <br />located. Emailed Keith Witkowski kwitkowski@ertsonline.com, requesting STLC <br />results. Received a response email that Keith is out of the office, email the same <br />request to pm@ertsonline.com. <br />Confirmation soil samples (Ground Sample G1-L Center, #G2-Left Top Corner, <br />and G3-Right Top Corner had detectable concentrations of several analytes. No <br />metal concentrations in the confirmation soil samples (collected from excavation <br />bottom) exceed the Title 22 concentrations set for hazardous waste determination. <br />TPH-g ranged from 0.05 mg/kg to 0.14 mg/kg. (C12-C22) ranged from 18 mg/kg <br />to <87.5 mg/kg, (C22-C32) ranged from 150 mg/kg to 302 mg/kg, and (C32-C40) <br />ranged from 202 mg/kg to 320 mg/kg. <br />06/11/2021 Received an email response in connection with STLC for lead (waste profiling) <br />from Keith Witkowski <kwitkowski@ertsonline.com>: "I have a call and email in to <br />Patriot Environmental Services to provide us/you with the requested info <br />Jamie.dobbs@patriotenvironmental.com <br />Amy.moore@patriotenvironmental.com <br />An update will be provided ASAP" <br />06/14/2021 Received an email from 'Keith Witkowski' <kwitkowski@ertsonline.com>; Jamey <br />Dobbs <Jamey.Dobbs@patriotenvironmental.com> with STLC results for waste <br />profiling. <br />SOIL BIN#20244 (STLC) = 1.6 mg/L and SOIL BIN#2 (STLC)= 1.48 mg/L. Base <br />on results no TCLP is needed for <br />SOIL BIN#20244 sample (non-hazardous) and TCLP is needed for further <br />determination for SOIL BIN#2 to determined for RCRA hazardous waste. <br />MANZO <br />MANZO <br />Sent an email to 'Keith Witkowski' <kwitkowski@ertsonline.com>; Jamey Dobbs <br /><Jamey.Dobbs@patriotenvironmental.com> asking for TCLP data. Received <br />several follow up phone calls from Keith, who indicated that based on STLC <br />results of < 5 mg/L TCLP was not ran for this sample. Sent an email to 'Keith <br />Witkowski' <kwitkowski@ertsonline.com>; Jamey Dobbs <br /><Jamey.Dobbs@patriotenvironmental.com with hazardous waste determination <br />code sections and recommendation to consult the EHD on future waste <br />determination events for spill clean up. Since the STLC result was <5 mg/L and <br />the STCL method and test duration is more stringent than TLCP the likelihood that <br />the TCLP result would be >5 mg/L is limited. <br />06/23/2021 Organized complaint paperwork, scanned to have a copy emailed to Zimmerman, MANZO <br />WIlliam@Waterboards <WIlliam.Zimmerman@Waterboards.ca.gov> per his <br />request.
