CONDITIONS/ASSUMPTIONS: • Customer will provide service water within 100 yards of the work area. • Customer will provide access to suitable,flat and stable location to stage and decontaminate equipment. • Waste pricing assumes twelve(12)ton minimum per load. • Patriot assumes that staging area with be fifty(50)feet from flammable materials. • Customer will provide right of entry to the premises and clear access to the work areas at the time(s)and locations(s)specified. • Additional work beyond the original scope of work,Time and Material(T&M)rates will apply according to the Patriot Rate Sheet. • Above unit costs are based on a non-union work force,no prevailing wages,no overtime work and no performance bond. • Above noted quantities are speculative. • With the exception of minimums,all billing will be based on actual quantities at the above unit costs. • Work will be performed during normal business hours(6:00 am to 6:00 pm),exlcuding federal and company holidays. • If work permits are required,they will be obtained and charged at cost plus twenty(20)percent. • Disposal cost is contingent on disposal facility approving the material,prior to delivery of the waste. • Patriot's pricing assumes all waste will be classified as non-hazardous. • If waste material is not accepted at the designated facility, Patriot will locate an approved alternative site for disposal. • Above costs include a standard recovery fee. • This proposal pricing is valid for thirty(30)days from the proposal date. •Patriot will be excused for any delay in completion of the project caused by Acts of God,acts of the client or the clients agent,weather conditions,health and safety concerns, labor disputes,acts of public utilities,regulatory agencies,additional work beyond the scope of the proposal,failure of the client to make payments promptly,or other contingencies unforeseen by Patriot and beyond the reasonable control of Patriot. If delays caused by the above mentioned factors do occur, performance period extension(s)or price adjustments equal to the delay time(s)shall be granted to Patriot,without liability to the client for liquidated damages due to that delay. •By signing the authorization to proceed,the Customer acknowledges that they have received,reviewed and agreed to the Standard Terms and Conditions(or the master service agreement between Customer and Patriot,if applicable).
