Pharmaceutical waste is not segregated for storage. <br />- Nonradioactive and non-RCRA pharmaceutical wastes, that are regulated as medical waste, shall be placed in a container or <br />secondary container labeled with the words "HIGH HEAT" OR "INCINERATION ONLY" on the lid and sides, so as to be <br />visible from any lateral direction. HSC 118275(a)(6)(B) – Pharmaceutical waste container in Linancia Pharmacy did not <br />have "High heat" or Incineration only" label on lid. <br />25. Biohazard bag is not placed in a rigid container that is leak resistant, tightly lid, clean, in good repair and appropriately <br />labeled. <br />- Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight -fitting cover, and be <br />kept clean and in good repair. Container shall be labeled with the words `Biohazardous Waste" or "BIOHAZARD" on the lid <br />and on the sides so as to be visible from any lateral direction. HSC 118280(c) – Biobazardous waste containers in 4th floor <br />soiled utility, surgery, and core lab were missing biohazard labels - Corrected on site. <br />29. Intermediate storage area is not secured or marked with proper signage. <br />- Intermediate storage area shall be either locked or under direct supervision or surveillance and marked with <br />the international biohazard symbol or the warning signs, "CAUTION—BIOHAZARDOUS WASTE <br />STORAGE AREA—UNAUTHORIZED PERSONS KEEP OUT", in English and in another language <br />determined to be appropriate by the infection control staff or LEA. HSC 118307 – Interim storage area <br />located on the outside of the "Short Stay Surgery" building did not have a lock on the perimeter gate. <br />Add lock to gate. <br />Unless otherwise noted, all violations noted above must be corrected by December 6th, 2016. <br />Notes: <br />1.) 2nd floor of hospital is currently under renovation. <br />2.) Hospital no longer has a labor and delivery section. <br />3.) Treatment and tracking records were available for review. <br />4.) Records of sterilization were available for review. <br />5.) Weekly spore tests were available for review. Ensure test results are accurately recorded. If changes need to be <br />made to recorded results, provide an explanation at the bottom of the page. <br />Reinspecion onlabout: A reinspection fee of $130 per hour may be charged.
