This registered large medical waste generator does not maintain individual treatment records and shipping and <br /> tracking documents for 2 years. <br /> -A large medical waste generator required to register with the EHD shall maintain individual treatment operation <br /> records, and shipping and tracking documents for all untreated medical waste shipped offsite for treatment for 2 years. <br /> HSC117975(a)–Log of the San I Pak alarms is not available for review. Maintain the log of alarms at the <br /> facility for review by the EHD. <br /> 24. Biohazardous waste is not placed in a biohazard bag that is conspicuously labeled. <br /> - Biohazardous waste shall be placed in a biohazard bag conspicuously labeled with words`Biohazardous Waste"or <br /> "BIOHAZARD". HSC 118275(a)(2)–The biohazardous container outside the soiled utility in the ER unit is <br /> missing biohazardous label. Make sure to have all biohazardous containers have the biohazardous label on <br /> them and visible. <br /> 24.Nonradioactive and non-RCRA pharmaceutical wastes, that are regulated as medical waste, shall be placed in a <br /> container or secondary container labeled with the words"HIGH HEAT OR INCINERATION ONLY"on the lid and <br /> sides, so as to be visible from any lateral direction. HSC 118275(a)(6)(B)-One of the pharmaceutical waste <br /> container in the Pharmacy is missing"Incinerator Only" label on it. Make sure that all pharmaceutical waste <br /> containers have"Incinerator Only"labels on all four sides and the top of the container. <br /> Tubing was coming out of one of the pharmaceutical waste container in the ER unit(soiled utility). Ensure that <br /> nothing is coming out of the container. <br /> 24. Trace chemotherapy waste is not segregated for storage. The secondary container is not labeled appropriately. <br /> - Trace chemotherapy waste shall be segregated for storage, and the secondary container shall be labeled with the <br /> words"Chemotherapy Waste"or"CHEMO"on the lid and sides, so as to be visible from any lateral direction. HSC <br /> 118275(a)(4)–The chemotherapy waste container in the Pharmacy located at Linacia Medical Center does not <br /> have any "chemotherapy waste" label on the container. Make sure to have the chemotherapy waste label on the <br /> chemotherapy waste container. <br /> 30. The designated accumulation area is not secured or marked with proper signage. <br /> -A designated accumulation area shall be secured by use of locks on entry doors, gates, or receptacle lids and <br /> marked with warning sign, "CAUTION—BIOHAZARDOUS WASTE STORAGE AREA—UNAUTHORIZED <br /> PERSONS KEEP OUT", in English and in another language determined to be appropriate by the infectious control <br /> staff or LEA. HSC118310–The biohazardous sign outside the soiled utility in the second floor is wearing <br /> down. Replace the biohazardous sign. <br /> Unless otherwise noted, correct all violations prior to April 9, 2019. <br /> Notes: <br /> 1. Pharmaceutical waste is being picked up by Stericycle every three to four days. <br /> 2.
