Time In: 8:25 am <br /> r <br /> Time Out: 8:38 am <br /> Food Program Complaint Inspection Report <br /> Name of Facility: PHO LUCKY RESTAURANT Date: 12/08/2021 <br /> Address: 8034 N ELDORADO ST, 95210 <br /> Owner/Operator: DOAN, DAVID Telephone: (209)472-9301 <br /> Program Element: 1600-FOOD PROGRAM Complaint#: C00054490 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> INFESTED WITH ROACHES.THEY ARE IN THE CUSTOMERS FOOD, THE TABLES,THE BAGS, THEY GAVE <br /> COMPLAINANT A BOX AND IT HAD ABOUT 30 ROACHES IN IT. COMPLAINANT PICKED UP AN ORDER FOR A <br /> CUSTOMER AND THERE WERE ROACHES ON THE CUSTOMERS FOOD.THIS HAS HAPPENED ON MORE THAN <br /> ONE OCCASION.THE FACILITY IS ALSO FILTHY. COMPLAINANT CALLED THEIR COMPANY TO LET THEM KNOW. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe.
