SAN '�d DAQ II II Environmental Health Department <br /> C0L.IJT`o' <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> NEW STAR PALLET 2150 E FREMONT ST, STOCKTON September 01, 2021 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 8 HSC 25505(a)(4) Failed to establish and electronically submit a reasonable or an adequate training program. <br /> OBSERVATION: The training program has not been annually certified/submitted as a part of the business plan.The <br /> training plan was not submitted for the year of 2018, 2019, 2020, and 2021.The training program has not been <br /> submitted since 5/11/2017.The training program was submitted late on 5/11/2017 for the year of 2017. <br /> REGULATION GUIDANCE: Per H&SC 25508.2, the business owner/operator shall annually review and certify that <br /> the business plan is complete, accurate, up-to-date and in compliance with Section 11022 of Title 42 of the United <br /> States Code.An annual electronic submittal satisfies these requirements.The annual business plan shall be <br /> submitted/certified by January 15 of each calendar year and may be submitted beginning November 1 of the <br /> previous year. <br /> CORRECTIVE ACTION: Immediately log into the CERS at http://cers.calepa.ca.gov/, review and submit/certify the <br /> current information. <br /> This violation was corrected on site. <br /> This is a repeat violation, Class II. <br /> 9 HSC 25508.2 Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br /> OBSERVATION: The business plan information has not been reviewed and resubmitted in the California <br /> Environmental Reporting System (CERS)annually by the due date, January 15. The business plan was submitted <br /> on 5/11/2017. The business plan was not submitted for the years of 2018, 2019, 2020, and 2021. <br /> REGULATION GUIDANCE: The hazardous materials inventory shall be submitted by January 15 of each calendar <br /> year and may be submitted beginning November 1 of the previous year. <br /> CORRECTIVE ACTION: Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated <br /> information, and submit to the EHD for approval. <br /> This violation was corrected on site. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> Conducted a Hazardous Materials Business Plan (HMBP) inspection. There were 6 violations found during the <br /> inspection. All violations have been corrected on site. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152).
