The Board of Equalization number is not valid. Also the facility <br />telephone number, (707) 431-3510, appears to be incorrect. Any change of information must be updated in CERS <br />within 30 days of the changes. Immediately log into CERS, update the required information, and submit for review by <br />the EHD. <br />UST Tank Information forms for the underground storage tanks (UST) T1, T2, T3, T4, and T5 are not current in CERS; <br />there is no Corrosion Protection identified in CERS.; T3, T4 and T5 - beneath Vent, Vapor Recovery (VR) and Riser/Fill <br />Pipe Piping Construction, remove "Fiberglass" from the Vapor Recovery Primary and Secondary Containment since <br />there is no vapor recovery for the diesel product. For T1, T2, T4 and T5 - beneath Vent, Vapor Recovery (VR) and <br />Riser/Fill Pipe Piping Construction, change Riser Pipe Primary Containment from Fiberglass to Steel. T4 and T5 - <br />beneath Tank Description, change Tank Configuration from "One in a Compartment Unit' to "A Stand-alone Tank' . <br />Beneath Overfill Protection, mechanical flappers are indicated by "Yes" for Fill Tube Shut-off valve; however, in the <br />Comments/Additional Information section of the Monitoring Plan, the statement "Overfill prevention is set at 90% of <br />tank capacity with audible and visual alarms." Clarify what method is used for overfill protection and revise CERS <br />accordingly. Any change of information must be updated in CERS within 30 days of the changes. Immediately log <br />into CERS, update the required information, and submit for review by the EHD. <br />This is a repeat violation, Class ll. <br />105 <br />CCR 2632 Monitoring and response plans not current or approved by the EHD. <br />The monitoring plan is not current and/or not approved by the EHD. For T1, T2, T3, T4, and T5, beneath Pipe <br />Monitoring is Performed Using the Following Methods, add "Yes" to Leak Alarm Triggers Automatic Pump Shutdown, <br />and "Yes" for Failure/Disconnect Triggers Pump Shutdown. Also beneath Recordkeeping, change "No" to "Yes" for <br />Equipment Maintenance and Calibration Records. The monitoring plan must be uploaded to the California <br />Environmental Reporting System (CERS). Immediately log into CERS, make the necessary changes, and submit for <br />review by the EHD. <br />This is a repeat violation, Class II. <br />106 <br />CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. <br />An accurate UST Monitoring Site Plan has not been submitted. A site plan must be submitted identifying the <br />locations where monitoring will be performed. Please include the Vacuum, Pressure, Hydrostatic (VPH) System <br />monitoring sensors in the site plan. Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/ and upload a copy of the UST Monitoring Site Plan. <br />This is a repeat violation, Class II. <br />Page 4 of 9 <br />
