CASE SUMMARY <br /> This is a gasoline and diesel dispensing facility consisting of two separate underground <br /> storage tank (UST) systems, each with its own monitoring panel. The truck stop side of <br /> the facility services tractor-trailer trucks with diesel fuel from two 20,000-gallon USTs. <br /> The automobile side of the facility services vehicles with 87-octane gasoline from a <br /> 20,000-gallon UST, and 91-octane gasoline and diesel from a 12,000/8,000-gallon, two- <br /> compartment UST, respectively. <br /> On May 2, 2007, Mr. Willy Ng, Senior Registered Environmental Health Specialist <br /> (RENS), San Joaquin County Environmental Health Department (EHD), witnessed <br /> testing of all sensors and spill containers at Fast Lane Central Valley, a new UST <br /> system. Leak detectors failed due to air trapped in the lines and required retesting after <br /> purging out the air. Mr. Ng did not leave an inspection report on site for this date. <br /> On May 8, 2007, Mr. Ng witnessed the testing of the leak detectors and gave approval to <br /> operate a new UST system. The permit application to install the LISTS incorrectly stated <br /> that the facility address was 316 E. Roth Road and that was the address written on the <br /> inspection report (Attachment 1). A test report was later submitted, but incorrectly noted <br /> test dates of May 2, 2007, and May 3, 2007 (Attachment 2). Enhanced leak detection <br /> testing was performed on March 22, 2007, and a report was submitted (Attachment 3). <br /> On June 23, 2008, Mr. Muniappa Naidu, Senior REHS, performed a routine UST <br /> inspection (Attachment 4) and found that the monitoring system certification, leak <br /> detector testing, and spill container testing had not been performed. Testing was last <br /> done in May 2007, and was due by May 2008. Current financial responsibility <br /> documents were not submitted to the EHD, and an alarm history log was not kept. Mr. <br />
