RENS <br />Fdim nlaeaec, REH6 <br />Re: FAST LANE CENTRAL VALLEY Si <br />116 ROTH RD <br />LAT14ROP, CA 95330 <br />The San Joaquin County Environmental Health Department (EHO) has completed the review of <br />the application for a permit to dean debris from auto side sump cellars and dean seal and <br />lubricate vapor sump covers, to adust overfill from 95% to 90%, and to replace sensors in UDC <br />14 and UDC 18 (Veeder Root 304, mird hydrostatic single point) on the underground storage <br />tank (UST) system at the above noted address. The permit has been approved votli the <br />following conditions <br />1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work An new addenda supersede the previous ones. <br />3. Curried Elena Manzo at (299) 053-7699 to arrange the following inspection <br />appointments (a 41 notice is required for inspection appointments). Please be <br />aware that inspections for emergency work must be scheduled within five days of permit <br />approval. <br />a Sensor functionality Functionality of the monitoring system must be verified to <br />ensure that monitoring has been restored to meet the requirements of Title 23 <br />California Code of Regulations and tire Health S Safety Code. If it is determined that <br />a cold start is necessary, a copy of the alarm history shall be printed prior to <br />peforming the "cold start' of tits Veeder Root system and made available at the time <br />of EID inspection. <br />In- overfill prevention. verification: Verify that external audblelvisual alarm for all USTs <br />activates when tanks are 9TA full. <br />c. Verify that all auto side sump collars (fill and STP) and vapor sump covers have <br />proper gaskets and are designed and installed to prevent water intrusion. Verify that <br />all secondary mntainment is product tight {secondary containment shall be <br />impervious to the liquid andvaporof the subsiance contained and a msbucted to <br />. prevent structural tveakeming as a result of contact with any hazardous substance <br />released from the primary containment). <br />
