The appointment with the EHD was for 9:00am. The testing did not begin <br />until 10:15am. Testing could not be completed on the29th due to time constraints. A checklist of violations was <br />left on site. <br />Inspector arrived on site on June 30, 2015 at the 9:00am scheduled appointment time. The testing did not begin <br />until almost 11:00am. Inspector witnessed testing by Al Milburn, technician who was on site yesterday with Brian <br />Dunahay. The certifications used yesterday were Brian's. <br />Inspector told Al she would need to see his certifications. He said ok. At completion of the inspection Al showed <br />inspector certifications. His ICC certification is current but the V/R expired 6/17/2015. Al stated he completed <br />course, need to take the online exam today after we were finished with inspection. Inspection agreed to accept <br />certification if it was completed today and had todays date on it. Inspector spoke with Brian and he confirmed he <br />would email a copy of AI's V/R certification tomorrow. If the certification is not valid for today the inspection <br />performed today will be considered void and will need to be redone at a later date. The re -inspection if necessary <br />will be billed at the current EHD hourly rate of $130/hour. If a re -inspection is required it must include all liquid <br />sensors, interstitial space sensors, vacuum sensors, overfill, fail safe and positive shut -down. <br />This inspection report supersedes report issued yesterday. <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by July 30, 2015. <br />Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br />documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST <br />Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer(if <br />applicable), and the Owner Statement of Designated Operator Compliance.
