A completed inspection report was faxed to the facility on July 30, 2010 <br /> (Attachment 35), and photos taken during the inspection were sent by mail on August 4, <br /> 2010 (Attachment 36). <br /> On July 29, 2010, a permit application, SR0060637, was submitted for the 91-octane fill <br /> sump sensor that was replaced during the previous day's inspection, and repair of the <br /> wiring for the 87-octane south UST annular sensor. Mr. Cacapit approved the permit on <br /> August 4, 2010 (Attachment 37). <br /> On August 30, 2010, a return to compliance certification and disposal records for the <br /> waste found on site during the July 26, 2010, hazardous waste inspection were <br /> submitted (Attachment 38). A corrective actions statement was not submitted. <br /> On September 1, 2010, Mr. Cacapit sent a fax to Mr. Khajevandi asking for a corrective <br /> actions statement for the violations on the July 26, 2010, hazardous waste inspection <br /> report (Attachment 39). <br /> On September 7, 2010, Mr. Cacapit sent a fax to Mr. Khajevandi asking for a corrective <br /> actions statement for the violations on the July 26, 2010, hazardous waste inspection <br /> report (Attachment 40). <br /> On September 14, 2010, a return to compliance certification, corrective actions <br /> statement, a September 8, 2010, work order for repairs to the leaking dispensers, and <br /> an updated designated operator employee training record were submitted (Attachment <br /> 41). <br /> On September 23, 2010, an amendment to permit application SR0060637 was <br /> submitted to replace the shear valve at dispenser 5/6. Mr. Cacapit approved the <br /> amendment the following day (Attachment 42). <br /> On October 13, 2010, Mr. Cacapit witnessed the successful testing of the 87-octane <br /> annular sensor (Attachment 43). The 91-octane annular sensor was replaced and <br /> tested successfully while on site, and the permit application would have to be amended <br /> to include this. Mr. Khajevandi submitted a recordable release statement to Mr. Cacapit <br /> for the diesel leaks for during the July 28, 2010, UST inspection (Attachment 44). <br /> On December 7, 2010, an amendment to permit application SR0060637 was submitted <br /> for the 91-octane UST annular sensor replaced on October 13, 2010 (Attachment 45). <br /> Mr. Cacapit approved the amendment that day. <br /> On April 22, 2011, a monitoring system certification test report for the testing performed <br /> on July 28, 2010, was submitted, eight months late (Attachment 46). <br /> On July 26, 2011, Ms. Michelle Henry, Senior REHS, EHD, performed a routine UST <br /> inspection (Attachment 47) and witnessed the monitoring system certification, leak <br /> detector testing, and spill container testing. The diesel leak detector failed when tested, <br /> and liquid was found in the vent sump. <br /> On August 3, 2011, a permit application, SR0063168, was submitted to replace the <br /> diesel leak detector. Mr. Cacapit approved the permit the following day (Attachment 48). <br /> 5 <br />
