OBSERVATION:A new designated operator,Amy Winkler, began conducting inspections on 9/3/2021 and notification was not provided to the EHD within 30 days of the change. REGULATION GUIDANCE: The owner or operator shall submit a"Designated Underground Storage Tank Operator Identification Form"no later than 30 days after any change in the designated UST operator(s). CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility. This is a minor violation. 208 CCR 2716(a-e) Failure to comply with the designated operator inspection requirements. OBSERVATION: The designated operator failed to check and note the correct test dates for the overfill prevention equipment testing on the 1/19/2021, 12/22/2020, 11/25/2020, 10/30/2020, and 10/1/2020 designated operator inspection reports. The designated operator failed to check and note the correct test date for the secondary containment testing on the 12/22/2020 and 10/1/2020 designated operator inspection reports. REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. ----------------------------------------------------- OBSERVATION: The designated operator failed to check and note all required items on the visual inspection reports.The incorrect and missing information included the following from the inspection reports: -Section 6 was not completely filled out on the 8/6/2021 monthly inspection report REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class ll.
