OBSERVATION: The following maintenance and monitoring records for the last three years were not found on site: 1. 12/11/2018, 12/3/2019, and 12/1/2020 Monitoring System Certification Forms and Spill Container Testing Report Forms. 2. 9/23/2021 Overfill Prevention Equipment Inspection Report Form REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring records include: (1)date and time of all monitoring or sampling; (2)monitoring equipment calibration and maintenance records; (3)results of any visual observations; (4)results of sample analysis performed a lab or in the field; (5)logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6)results of inventory readings and reconciliations. CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the last three years on site and submit copies to the EHD. This is a repeat violation, Class II. 208 CCR 2716(a-e) Failure to comply with the designated operator inspection requirements. OBSERVATION: The designated operator failed to check and note the correct test dates for the 9/23/2021 overfill prevention equipment inspection and 8/5/2020 secondary containment testing on the 12/14/20, 1/13/21, 2/12/21, 3/12/21,4/9/21, 5/7/21, 6/6/21, 7/6/21, 8/3/21, 9/1/21, 9/30/21, 10/26/21, and 11/24/21 designated operator inspection reports.The overfill prevention equipment inspection test date was incorrectly noted as 12/11/18 and the secondary containment test date was incorrectly noted as 8/4/20. REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. ------------------------------------------------------------------------------------------- OBSERVATION: Designated operator reports were not signed within the required time frame. -The 12/14/20, 1/13/21, 2/12/21, 3/12/21,4/9/21, 5/7/21, 6/6/21, 7/6/21, 8/3/21, 9/1/21, 9/30/21, 10/26/21, and 11/24/21 designated operator reports were not signed by the owner/operator. REGULATORY GUIDANCE:Within 48 hours of the completion of the designated UST operator visual inspection, the designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 72 hours of being provided a signed copy of the report,the owner or operator shall provide a description of each corrective action taken or to be taken and shall sign and date the report, acknowledging the identified compliance issues. CORRECTIVE ACTION: Ensure all designated operator inspection reports are completed and signed within the required time frame in the future. This is a repeat violation, Class II.
