COMPLAINANT ALLEGES THAT <br /> THERE WAS A COCKROACH BAKED IN TO THE PIZZA. (ON TOP OF THE CHEESE/BETWEEN MEAT) <br /> COMPLAINANT CONTACTED FACILITY. COMPLAINANT PROVIDED TWO PHOTOS." <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> General Manager(GM)is on site and is not aware of the compliant. Per GM, the facility does not have an issue with <br /> cockroaches. Facility has a pest control company that services routinely. <br /> Upon inspection this date, there were no cockroaches or insects within the facility.\ <br /> Observed a service report from Next Generation Pest Control.The most recent service date is 11-18-21.
