S0 JOA U 1 Environmental Health Department <br /> U ; i y <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> FARWEST SAFETY INC 226 N MAIN ST, LODI September 29, 2021 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 303 CCR 66262.34(d)(2) Failed to maintain all communication or alarm systems, spill control, or decontamination <br /> equipment. <br /> OBSERVATION: An eye wash bottle was observed in the sign shop with an expiration date of May 2021. <br /> REGULATION GUIDANCE:All facility communications or alarm systems,fire protection equipment, spill control <br /> equipment, and decontamination equipment,where required, shall be tested and maintained as necessary to assure <br /> its proper operation in time of emergency. <br /> CORRECTIVE ACTION: Bring the eye wash bottle into proper working order by repair, replacement or testing. <br /> Provide a corrective action statement to the San Joaquin County Environmental Health Department within 30 days. <br /> This is a Class II violation. <br /> 3010 OBSERVATION: Per operations manager John Ashe, approximately 3,200 pounds of treated wood waste was <br /> disposed approximately 1 week ago. <br /> REGULATION GUIDANCE: California Health and Safety Code section 25230.8 <br /> (a)A treated wood waste handler shall keep a record of each shipment of treated wood waste sent from the handler <br /> to treated wood waste facilities.The record may take the form of a log, invoice, manifest, bill of lading, shipping <br /> document, or receipt from a treated wood waste facility. The record for each shipment of treated wood waste shall <br /> include all of the following information: <br /> (1)The name and address of the treated wood waste facility to which the treated wood waste was sent. <br /> (2)The weight of treated wood waste,the estimated weight of treated wood waste, or the weight of the treated wood <br /> waste as measured by the receiving treated wood waste facility.An estimated weight may be used when a scale is <br /> unavailable or weighing is impractical.Assumptions required for weight estimates shall be recorded in the shipment <br /> records. <br /> (3)The date the shipment of treated wood waste left the handler. <br /> CORRECTIVE ACTION: Provide treated wood waste disposal records within 30 days. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> An inspection checklist was provided to the facility operator at the time of inspection.
