SAN J 0 A Q U 1 N Environmental Health Department <br /> U ; i y 1 Iii <br /> CA Accidental Release Prevention Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> MOUNTAIN HOUSE CSD WATER TREATMENT 1 18045 S KELSO RD, BYRON September 29, 2021 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 509 CCR 2755.2(e) Failed to document the results of the HR and/or ensure that problems identified were resolved. <br /> OBSERVATION: The owner/operator failed to document the results of the hazard review and ensure that problems <br /> identified are resolved. <br /> Hazard review was conducted on 1/9/2019, recommendations are due 2.5 years from when the hazard review was <br /> initially conducted. Out of the 36 recommendations, 31 are missing completion dates. <br /> REGULATION GUIDANCE: The owner or operator shall document the results of the hazard review and ensure that <br /> problems identified are resolved. The owner or operator shall enter into an agreement with the Unified Program <br /> Agency on a timetable for resolution of these problems. Otherwise these resolutions shall be completed within two <br /> and one-half(2.5)years of performing the hazard review or the next planned turnaround for items requiring a <br /> turnaround. These timelines shall not apply to any hazard review completed prior to January 1, 2015. The final <br /> resolution taken to address the hazard review recommendation and the actual completion date shall be <br /> documented. <br /> CORRECTIVE ACTION: Submit documentation to our department demonstrating that the hazard review <br /> recommendations have been or will be resolved. <br /> This is a Class II violation. <br /> 510 CCR 2755.2(f)Failed to update and revalidate the HR at least once every five years. <br /> OBSERVATION: The owner/operator failed to update and revalidate the hazard review at least once every five <br /> years. <br /> Last hazard review was conducted 11/7/2013, next hazard review was conducted 1/9/2019. Over the 5 year <br /> timeframe. <br /> REGULATION GUIDANCE: The hazard review shall be updated and revalidated at least once every five years. <br /> The owner or operator shall also conduct reviews whenever a major change in the process occurs.
