S0 JOA U 1 Environmental Health Department <br /> U ; i y <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Custom Design Manufacturing 248 E Kettleman Ln, Lodi September 30, 2021 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 115 CCR 66266.81(a)(4)(B)Failed to retain copy of manifest or bill of lading for spent lead acid batteries for 3 years. <br /> OBSERVATION: Bills of lading for the recycling of lead acid batteries from 09/30/2018 through 09/30/2021 were <br /> not found on site. <br /> REGULATION GUIDANCE: Title 22 California Code of Regulations section 66266.81 Requirements <br /> CORRECTIVE ACTION: Provide copies of battery core bill of lading records and a corrective action statement to <br /> the San Joaquin County Environmental Health Department within 30 days. <br /> This is a Class II violation. <br /> 301 CCR 66262.34(d)(2) Facility not maintained to minimize the release of a hazardous waste. <br /> OBSERVATION: A spill was observed under a vehicle in the shop and has not been cleaned up. <br /> REGULATION GUIDANCE:A hazardous waste generator must maintain and operate its facility to minimize the <br /> possibility of a fire, explosion, or any unplanned sudden or non-sudden release of hazardous waste or hazardous <br /> waste constituents to air, soil, or surface water which could threaten human health or the environment. <br /> CORRECTIVE ACTION: Immediately clean the spill. Manage the waste according to Title 22 California Code of <br /> Regulations for hazardous waste. Submit a statement and supporting documentation to the San Joaquin County <br /> Environmental Health Department describing the storage and final disposition of the cleaned up waste. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by October 30, 2021 . <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152).
