Hazelton Avenue <br /> 4 <br /> Stockton , California 95205 <br /> 5 <br /> 6 <br /> 7 A photocopy of the check shall be sent to the following CUPA personnel assigned to this <br /> 8 <br /> matter , Michelle Henry , San Joaquin County Environmental Health Department <br /> 9 <br /> 10 If Respondent fails to make payment as provided above , Respondents agrees to pay interest <br /> 11 at the rate established pursuant to HSC § 25360 . 1 and to pay all costs incurred by the CUPA <br /> 12 in pursuing collection including attorney ' s fees . <br /> 13 <br /> OTHER PROVISIONS <br /> 14 <br /> 15 12 . 1 , Additional Enforcement Actions : By agreeing to this Consent Order , the <br /> 16 <br /> CUPA does not waive the right to take further enforcement actions , except to the extent <br /> 17 <br /> provided in this Consent Order . <br /> 18 <br /> 19 12 . 2 . Penalties for Noncompliance : Failure to comply with the terms of this <br /> 20 Consent Order may subject Respondents to civil penalties and /or punitive damages for any <br /> 21 <br /> costs incurred by the CUPA or other government agencies as a result of such failure , as <br /> 22 <br /> 23 provided by HSC § 2540491 . 1 and other applicable provisions of law . <br /> 24 12 . 3 .
