COMPLAINT INSPECTION REPORT <br />12:25 pm <br />12:00 pm <br />Time Out: <br />Time In: <br />Environmental Health Department <br />Name of Facility: DOSA BIRYANIZZ CHAAT CAFE Date: 09/09/2021 <br />Address: 2805 NAGLEE RD, TRACY 95304 <br />Owner/Operator: Telephone: (209) 319-8189 <br />Program Element: 1600 - FOOD PROGRAM Complaint #: CO0054223 <br />Responsible Party Contact: <br />DESCRIPTION OF COMPLAINT ALLEGATIONS <br />The San Joaquin County Environmental Health Department received the following complaint: <br />CO0054223 states: "ON AUGUST 23RD AROUND 8:30PM; COMPLAINANT ORDER GOAT CURRY. WHEN <br />COMPLAINANT WENT TO EAT THE FOOD AFTER A FEW BITES HIS MOUTH STARTED GOING NUMB. HE CALLED <br />FACILITY TO FIND OUT WHAT WAS IN FOOD, WHICH HAD COCONUT; COMPLAINANT VERY ALLERGIC TO <br />COCONUT, HE WAS GIVEN WRONG FOOD. WHEN HE COMPLAINED TO RESTAURANT ABOUT WRONG FOOD <br />BEING ALLERGIC TO SOME OF IT; EMPLOYEE / HOSTESS WAS RUDE AND SHE TOLD COMPLAINANT IT WAS " <br />NOT THERE PROBLEM" COMPLAINANT WOULD LIKE A CALL BACK AFTER INSPECTION. " <br />VIOLATIONS AND CORRECTIVE ACTIONS <br />Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br />113700. All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodborne illness. All major violations must be corrected immediately. Non-compliance may warrant immediate closure of <br />the food facility. <br />OVERALL INSPECTION NOTES AND COMMENTS <br />No Temperature Data Collected <br />FOOD ITEM -- LOCATION -- TEMP º F -- COMMENTS <br />NOTES <br />Manager is on site and is aware of the complaint. Manager states that the customer asked what type of curry the facility <br />offers. The Manager told the customer. The customer called back to say that the wrong order was received, but the customer <br />never stated that he/she had a coconut allergy in the two telephone conversations that took place. <br /> Manager states that both the goat curry and the goat chettinad dishes have coconut in them. Both dishes have the goat meat, <br />spices, coconut (or coconut milk) , onion sauce and tomato paste. The coconut milk is used as a thickening agent and to <br />bring down the spice level. <br />No signature obtained <br />Report typed at the office 3:07pm-3:25pm <br />Manager states that the customer wrote a poor review about the facility as a result. <br />The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br />Safety Code.
