SANAOAQUIN <br />----COUNTY <br />Grrtnc <br />Environmental Health Department <br />Time In: 12:00 pm <br />Time Out: 12:17 pm <br /> <br />COMPLAINT INSPECTION REPORT <br />Name of Facility: RICE & SPICE Date: 09/22/2021 <br />Address: 1030 S HUTCHINS ST, LODI 95240 <br />Owner/Operator: MAZHAR ALI Telephone: (209) 333-2980 <br />Program Element: 1600- FOOD PROGRAM Complaint #: C00054283 <br />Responsible Party Contact: <br />DESCRIPTION OF COMPLAINT ALLEGATIONS <br />The San Joaquin County Environmental Health Department received the following complaint: <br />COMPLAINANT ALLEGES THAT THERE IS A HORRIBLE SMELL INSIDE THE STORE. COMPLAINANT STATES THAT <br />IT SMELLS LIKE SOMETHING IS DEAD AND ROTTING AND STARTING TO PRODUCE GASES. COMPLAINANT <br />STATES THE FURTHER YOU GO BACK IN THE STORE, THE WORSE THE SMELL GETS, THEY HAD TO LEAVE THE <br />STORE BECAUSE THEY BECAME DIZZY AND NAUSEOUS AND AFTER THEY LEFT, THEY VOMITED ON THE SIDE <br />OF THE ROAD. COMPLAINANT BELIEVES THEY COULD BE USING A CHEMICAL LIKE CYANIDE TO KILL RATS. <br />VIOLATIONS AND CORRECTIVE ACTIONS <br />Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br />113700. All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodbome illness. All major violations must be corrected immediately. Non-compliance may warrant immediate closure of <br />the food facility. <br />OVERALL INSPECTION NOTES AND COMMENTS <br />FOOD ITEM -- LOCATION --TEMP ° F -- COMMENTS <br />No Temperature Data Collected <br />NOTES <br />I spoke to owner who was not aware of compliant, however per owner approximately two weeks ago he had to "kick a <br />customer out" of the facility. <br />Per owner the customer wanted a refund after 3 days of purchasing a pizza. <br />Per owner, spray to kill rats is is not used at the facility. <br />I did not notice a "horrible smell" at this time. <br />• <br />The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br />Safety Code.
