Per Alex, approximately 10 gallons of mineral oil spilled from a pole <br />mounted transformer due to a vehicular accident. Prepared a notice to abate and <br />emailed report to R2SV@pge.com <br />VAN G-LEE <br />07/22/2021 Prepared Prop 65. VANG-LEE <br />07/23/2021 Went to site to verify that spill is secured. Spill is not secured. Spill is about thirty <br />feet from roadway against a fence. Spill area covers an approximate area of 5x2 <br />feet. Left site. <br />BAKER <br />Completed photo log. <br />07/24/2021 12:12pm-Received call from Dave Larsen of PG&E that clean-up is ready to be <br />verified. <br />BAKER <br />12:24pm-Left for site. Arrived on site at 1:55pm. Excavation was approximately <br />4x4 feet in area and 3-4 feet deep. Clean-up is complete. Waste was put in a 20 <br />yard roll off and will be taken to Forward landfill. <br />Site was secured by PG&E. The phone company came later to fix the phone line <br />and took all the cones. <br />1:40pm-Left site <br />2:15pm-Completed photo log. <br />12:12pm-2:38pm. <br />08/09/2021 Contacted Alex Steele of PG&E to follow-up regarding disposal records for the <br />waste generated from the site clean-up. Alex states that the waste was taken to <br />VANG-LEE <br />4040 West Lane, Stockton, CA 95204 and will be disposed of at a later time. <br />08/12/2021 Entered a note in PR0513829 to follow-up with disposal records for waste <br />generated during clean-up.
