Joint inspection with CUPA (Routine WONG <br />PR0514120) Garrett Backus. Industrial waste stream is captured, but waste from <br />cleaning of equipment which included acid is and had been discharged to sink on <br />the second floor and to the existing septic. Told manager cease immediately and <br />also to implement an engineering solution so no waste other than sewage can be <br />released to the septic system. Told manager I will generate the NAI at a later time <br />due to schedule conflicts, but expect a reinspection in about a month's time, <br />possbily toward end of July. Told GB of CUPA and also M Henry I will also send a <br />copy to the property owner. MH recommend I call the property owner; told her I <br />will but I don't have the phone number at this time. <br />06/17/2021 Wilcox 2470 Formurex - Generated complaint (Post inspection paperwork from WONG <br />6/15) but still unable to find the time to complete the NAI <br />06/24/2021 Wilcox 2470. Completed Formurex NAI from 6/15 inspection. Mailed by certified WONG <br />to both property owner and Formurex. See also concurrent CUPA inspection <br />report in LF under PRO514120. Located property owner's phone numbers, but <br />when called: 981-7374, no voice box was set up, and cell 298-7505 out of service <br />when called <br />06/25/2021 Wilcox 2470 Formurex. Attempted to contact with the property owner at the WONG <br />2residential buildings (+1 mobile home) on site based on rumor that they live <br />there. No one at any the buildings. Non billed. <br />06/25/2021 Wilcox 2470 Formurex. Non Billed time. Spoke to Michelle H. and told her I have WONG <br />not been able to reach the property owner by phone to date: Mr. Chandler's cell is <br />out of service, and his normal phone has not been set up for voice mail. Informed <br />MH that the certified letter had been mailed though I have not received the receipt <br />back yet. Agreed to try to contact the property owner. Managed to reach Louise <br />(609-8015) for the property management company <br />(curryswarehouse@yahoo.com).
