Complaint Investigation Summary Report #: 5105 COMPLAINT ID: C00054009 Site Location: 1121 S ANNABELLE LN Activity Summary Activity Date Recorded by 09/30/2021 Arrived at the property for the scheduled septic destruction and no one was there. SUSZYCKI Waited for about 10 minutes and called Golden Opportunity. They told me that they would contact the property manager. A man come out from behind one of the homes and he did not speak English but was able to the gate for me. Once inside, I could see that the septic had been drained but was still not destroyed. I informed the property manager that the septic needs to be crushed and filled in with dirt for me to sign off on the permit. See SR0083965. Received a call back from Golden one asking if everything was taken care of. I told them that the septic actually needs to be destroyed for me to sign off on the septic destruction permit. I was told that I would get a call when it was taken care of. 10/04/2021 Discussed complaint/case with Michael S. They still haven't called to schedule the SANGALANG septic destruction inspection. I emailed Kary Zhang to get an update. Received an email back from Kary. The contractor is still working on it. Kary will update me as soon as there is an update. 10/06/2021 Golden one called and asked what needed to be done with the septic. I explained SUSZYCKI to them in detail that the demolition requires the septic to crushed and filled in. I also said that once they are done to let me know so I can arrange to inspect the demolition. 10/07/2021 Received a call from the contractor for the septic , He stated that the septic is destroyed and ready for inspection. I told him that I would come out there to inspect it. I set up an appointment for 10/8/21 at 1000 see SR0083965. SUSZYCKI 10/08/2021 Septic destruction permit has been finaled.
