S A N J D AR I N Environmental Health Department <br /> ij ;'�J i Y <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: FDat,-.ARCO AM PM#82602* 2430 JOE POMBO PKWY, TRACY ber 21, 2021 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br /> OBSERVATION: The site map is incomplete. <br /> -internal roads were not listed on the map <br /> -fuel and carbon dioxide loading areas were not listed on the map <br /> -2 storm drains were observed on site that were not listed on the map <br /> -2 fire extinguishers were observed on site that were not listed on the map <br /> REGULATION GUIDANCE:A site map shall contain a north orientation, loading areas, internal roads, adjacent <br /> streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as"NA". <br /> CORRECTIVE ACTION: This was corrected on site.An updated map was uploaded into CERS during the <br /> inspection <br /> This is a repeat violation, Class II. <br /> 9 HSC 25508.2 Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br /> OBSERVATION: The business plan information has not been reviewed and resubmitted in the California <br /> Environmental Reporting System (CERS)annually by the due date, January 15. <br /> -The 2021 business plan submittal was on time on 12/8/2020 but incomplete(missing sections: hazardous <br /> materials inventory and emergency response and training plans).A complete submittal was made on 9/1/2021 but <br /> was late. <br /> -The 2020 business plan submittal was on time on 11/14/2019 but incomplete(missing sections: hazardous <br /> materials inventory and emergency response and training plans). <br /> -The 2019 business plan submittal was late on 10/17/2019 and was incomplete(missing sections: hazardous <br /> materials inventory and emergency response and training plans). <br /> REGULATION GUIDANCE: The hazardous materials inventory shall be submitted by January 15 of each calendar <br /> year and may be submitted beginning November 1 of the previous year. <br /> CORRECTIVE ACTION: Ensure business plan submittals(Facility Information, Hazardous Materials Inventory, and <br /> Emergency Response and Training Plans)are submitted annually by January 15. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> Conducted a Hazardous Materials Business Plan (HMBP) inspection. There were two violation(s) during the <br /> inspection. Complete and submit a copy of the Return to Compliance Certification form to the EHD with a <br /> statement documenting the corrective actions that have been or will be taken for each violation, and any <br /> supporting paperwork, by 11/20/2021. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign.
