No shipping records <br />were available for review. <br />The facility collects waste electronic devices and batteries and takes them to a recycling facility but the recycling <br />facility was not identified. <br />REGULATION GUIDANCE: <br />Title 22 California Code of Regulations section 66273.39. Tracking Universal Waste Shipments <br />Title 22 California Code of Regulations section 66273.8 Exemptions <br />CORRECTIVE ACTION: <br />Locate a copy of all dental amalgam shipping records from February 9, 2018 to February 9, 2021, maintain them on <br />site and provide a copy to the San Joaquin County Environmental Health Department (EHD). Also provide <br />documentation that the dental amalgam is relinquished to a universal waste handler, transporter or destination <br />facility. <br />Determine which recycling facility that electronic devices and batteries are taken to and provide that information to <br />the EHD. Also provide the amount of electronic devices and batteries that are recycled yearly. <br />This is a Class II violation. <br />3010 OBSERVATION: The facility processes x-rays using ImageMax Developer and Fixer Concentrate. One receipt, <br />dated October 2020, shows the spent Developer and Fixer being hauled off site by Stericycle. The receipt does not <br />show that the spent Fixer is being sent to a facility for silver recycling. Provide <br />documentation showing the destination of the photoimaging solution waste and how it is being recycled for silver <br />recovery or where it is going for disposal.
