`AiA Facility/Permit Number: <br /> SOLID �Vl <br /> WASTIE C FACIL IT PERMIT' <br /> 39-AA-020 <br /> 16. Self Monitoring: <br /> a. Results of all self-monitoring programs as described in the Report of Facility Information,will be reported as follows: <br /> Program Reporting Facility Agency Reported To <br /> 1. Log of Special Occurrences(.e.tires,explosions, ANNUALLY LOCAL ENFORCEMENT AGENCY <br /> accidents,inadvertent acceptance of hazardous wastes, (LEA) <br /> etc...)must be kept on site and submitted to LEA annually. <br /> 2. An employee training log with course descriptions,dates of KEEP ON SITE AND AVAILABLE LEA <br /> training,etc...shall be maintained and kept current. UPON REQUEST OF LEA <br /> 3. A record of weather conditions shall be maintained with UPON REQUEST OF LEA LEA <br /> operational records pursuant to 14 CCR Section 17875. <br /> 4. A report of quantities and types of wastes received and LEA <br /> product removed from the facility shall be provided to the QUARTERLY <br /> LEA on a quarterly basis(showing monthly breakdowns). <br /> The records shall indicate the quantities in tons/cubic yards <br /> and include weight and record data. Records shall be <br /> submitted to the LEA on the 15th day of the subsequent <br /> month. <br /> 5. Temperature of the active compost shall be monitored and LEA <br /> recorded as specified in the California Code of Regulations, UPON REQUEST OF LEA <br /> Title 14,Division 7,Articles 3&4(14CCR). Records <br /> shall be kept for kinds and amounts of additives/ <br /> amendments used in composting process. Whatever <br /> composting method used all records pertinent to the <br /> particular operation are required to be kept current on site <br /> as per 14CCR Section 17887. <br /> 6. In addition to storm water monitoring records a log shall be LEA,Regional Water QualityControl Board <br /> kept for leachate monitoring. One sample of leachate shall QUARTERLY g (RWQC ) <br /> be taken from each of the first five composting batches of <br /> feedstock generating leachate and analyzed for organo- <br /> phosphates,using EPA method 8140,plus nitrate testing <br /> (14CCR,Section 17876.(A)i) <br /> 7. One sample per 5000 cy of finished compost must be taken <br /> and analyzed at a Department of Health Services Approved MONTHLY LEA <br /> Laboratory. Samples shall be analyzed for constituents <br /> required pursuant to 14 CCR,Section 17887. <br /> S. A certification that pathogen reduction criteria are met must <br /> be completed using CIWMB Form 5000. <br /> LEA <br /> 9. Submit an annual report to the LEA by January 31,of each ANNUALLY <br /> calendar year summarizing all reporting requirements. <br />
