Environmental Health Department <br />Hazardous Materials Program Inspection Report <br />Date: <br />November 22, 2021 <br /> Facility Address: <br /> 653 E PINE ST, LODI <br /> Facility Name: <br /> PR'S AUTOHAUS OF LODI <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br />OBSERVATION: The site map is incomplete. The following required items were missing: <br />-loading areas <br />-internal roads <br />-storm/sewer drains <br />-locations of hazardous materials <br />-emergency response equipment <br />-emergency shut-offs <br />-all access/exit points <br />REGULATION GUIDANCE: A site map shall contain a north orientation, loading areas, internal roads, adjacent <br />streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br />material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as “NA". <br />CORRECTIVE ACTION: This violation was corrected on site. <br />This is a repeat violation, Class II. <br /> 9 HSC 25508.2 Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br />OBSERVATION: The business plan information was not reviewed and resubmitted in the California Environmental <br />Reporting System (CERS) annually by the due date, January 15. The facility did not make a submittal in 2019. <br />REGULATION GUIDANCE: The hazardous materials inventory shall be submitted by January 15 of each calendar <br />year and may be submitted beginning November 1 of the previous year. <br />CORRECTIVE ACTION: This violation was corrected with the on-time 2020, 2021, and 2022 CERS submittals. <br />This is a repeat violation, Class II. <br />Overall Inspection Comments: <br />Conducted a Hazardous Materials Business Plan (HMBP) inspection. There were 2 violations during the <br />inspection. All violations were corrected at the time of the inspection. <br />To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br />of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br />($152).
