SAN J 0 A Q I I I Irl Environmental Health Department <br /> CCUNTY <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Herzog Transit Services Inc. 1020 E Alpine Ave, Stockton December 01, 2021 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br /> OBSERVATION: The following were found on site that have not been reported to CERS: <br /> -One additional 55 gallon drum of neutral floor cleaner observed at S&I track <br /> -Three additional 55 gallon drums of glass cleaner observed at S&I track <br /> -One additional 55 gallon drum of detergent disinfectant observed at S&I track <br /> REGULATION GUIDANCE:Any material that meets or exceeds the reportable quantity shall be reported in the <br /> facility's business plan. <br /> CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for <br /> approval. <br /> This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br /> OBSERVATION: The site map is incomplete. The following items are missing: <br /> -all hazardous materials need to be identified on the site map <br /> -loading areas <br /> -storm and sewer drains <br /> REGULATION GUIDANCE:A site map shall contain a north orientation, loading areas, internal roads, adjacent <br /> streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as"NA". <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class 11. <br /> 9 HSC 25508.2 Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br /> OBSERVATION: The business plan information has not been reviewed and resubmitted in the California <br /> Environmental Reporting System (CERS)annually by the due date, January 15. <br /> -2019 (submitted late on 5/5/2019) <br /> -2020 (not submitted) <br /> -2021 (submitted late on 1/25/2021) <br /> REGULATION GUIDANCE: The hazardous materials inventory shall be submitted by January 15 of each calendar <br /> year and may be submitted beginning November 1 of the previous year. <br /> CORRECTIVE ACTION: This was corrected on 1/25/2021. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> FA0022230 PR0538716 SCO01 12/01/2021 <br /> EHD 19-01 Rev.9/16/2020 Page 2 of 3 Hazardous Materials Program OIR <br /> 1868 E.
