The attached two-part questionnaire is designed to supplement the interview <br />requirement of the SSCR. <br />Part 1 includes questions which relate specifically to the user's responsibilities, and to additional <br />information sources which will be useful to us in completing our investigation most efficiently. <br />Part 2 includes questions to be asked of the current property owner and/or occupants. <br />Please complete the questionnaire at your earliest convenience and return it to us promptly. <br />We thank you for your assistance with this process.
