Facility/Permit Number: <br /> SOLID WASTE FACILITY PERMIT 39-AA-004 <br /> 16. Self Monitoring: <br /> a. Results of all self-monitoring programs as described in the Report of Facility Information,will be reported as follows: <br /> Program Reporting Facility Agency Reported To <br /> LOCAL ENFORCEMENT AGENCY <br /> 1. A report of quantities and types of wastes received and QUARTERLY (LEA) <br /> product removed from the facility shall be provided to the <br /> LEA on a quarterly basis(showing monthly breakdowns). <br /> The records shall indicate the quantities in tons/cubic yards <br /> and include weight and record data. Records shall be <br /> submitted to the LEA on the 15th day of the subsequent <br /> month. <br /> 2. Submit tonnage records of wastes received on a monthly MONTHLY LEA <br /> basis to LEA. <br /> 3. An employee health and-safety training log with course UPON REQUEST OF LEA LEA <br /> descriptions,names of employees trained,and dates of <br /> training,shall be maintained and kept current. <br /> 4. Log of Special Occurrences(i.e.tires,explosions, ANNUALLY LEA <br /> accidents,inadvertent acceptance of hazardous wastes, <br /> etc...)must be kept on site and submitted to LEA annually. <br /> 5. Environmental measurements of water quality,leachate,and QUARTERLY LEA, <br /> gas,shall be reported to the LEA on a quarterly basis. REGIONAL WATER QUALITY <br /> CONTROL BOARD(RWQBC) <br /> 6. Submit an annual report to EHD by January 31,of each ANNUALLY LEA <br /> calendar year summarizing all reporting requirements(i.e. <br /> methane gas,monitoring groundwater monitoring,and all <br /> other appropriate agency required monitoring. <br />
