The application addresses changes in operation at <br />FSL necessary to handle increased waste tonnages and to perform <br />recycling activities at the site. <br />On June 26, 1995 EHD sent the CIWMB a Draft Revised Permit <br />Application accompanied with pertinent information for this <br />operation. EHD approved the permit proposal (letter dated June 26, <br />1995) with conditon that a Periodic Site Review (PSR) was received <br />for FSL. <br />A PSR for FSL was received by EHD on August 2, 1995. This document <br />was reviewed and found to contain all necessary information for the <br />site as required. <br />EHD approves the Proposed Revised Permit (PRP) for FSL as enclosed. <br />Some changes have been made on the PRP and further site information <br />was received by EHD since the Draft Revised Permit was sent. All <br />recent information including a map showing the recycling location <br />area and a copy of the contract agreement is being forwarded to the <br />CIWMB at this time. <br />A Division of San Joaquin County Health Care Services <br />
