After the review the <br />EHD has determined that the complete and correct finding could not be made and is therefore <br />rejecting the application and JTD. Listed below are comments that should be addressed prior to <br />resubmitting the permit application. <br />1. The permit application Part 1. General Information does not indicate that a JTD <br />amendment was part of the application. Revise to include this information. <br />2. The permit application Part 2. Facility Description indicates that the zip code for the <br />facility is 95206 (actual zip code is 95236). Revise to include this inforination. <br />3. The permit application Part 2. E. Types of Wastes Accepted; the application does not <br />reflect that the facility accepts large dead animals. There is no restriction on dead animals <br />at this facility and this provides a valuable service to the community. Revise to reflect <br />actual site activities. <br />4. The permit application Part 3. A. Proposed Change; there are changes in the facility <br />operations as described in JTD. Revise to include a brief description of these activities in <br />this area. <br />5. The permit application Part 5. A. Negative Declaration; the State Clearing House (SCH) <br />number does not match the number list on the current facility permit. Check the number <br />and revise if necessary. <br />6. The permit application Part 6. List of Attachments A. Required With All Application <br />Submittals; the Use Permit number listed in the facility permit is not provided. Revise to <br />include this information. <br />7. The permit application Part 6. B. Additional Required Documents for Landfills Only; <br />Financial Liability Financial Mechanism is checked, but the method is not listed. Revise <br />to include this information. <br />8. The permit application Part 6. B. Closure Post Closure Maintenance Plan is checked, but <br />the date is not listed. <br />
