Self Monitoring: <br /> a.Results of all self-monitoring programs as described in the Report of Facility Information,will be reported as follows: <br /> Program Reporting Frequency <br /> 1. A report of quantities and types of wastes received and product removed from the facility Quarterly to the LEA <br /> shall be provided to the LEA on a quarterly basis(showing monthly breakdowns). The <br /> records shall indicate the quantities in tons/cubic yards and include weight and record data. <br /> Records shall be submitted to the LEA on the 15'h day of the subsequent month. <br /> 2. Submit tonnage records of wastes received on a monthly basis to the LEA. Monthly to the LEA <br /> 3. An employee health and safety training log with course descriptions,names of employees Upon request of the LEA <br /> trained,and dates of training,shall be maintained and kept current. <br /> 4. Log of Special Occurrences(i.e.tires,explosions,accudents, inadvertent acceptance of Annually to the LEA <br /> hazardous wastes,etc...)must be kept on site and submitted to LEA annually. <br /> 5. Environmental measurements of water quality, leachate,and gas shall be reported to the LEA Quarterly to the LEA <br /> on a quarterly basis. and the RWQCB <br /> 6. Submit an annual report to the EHD by January 31,of each calendar year summarizing all Annually to the LEA <br /> reporting requirements(i.e. methane gas,monitoring groundwater monitoring,and all other <br /> appropriate agency required monitoring.) <br />
