<br />Environmental Health Department SAN JOAQUIN <br />Time In: 9:00 am <br />Time Out: 9:20 am <br />COMPLAINT INSPECTION REPORT <br />Name of Facility: MANGY MOOSE CAFE Date: 12/16/2021 <br />Address: 506 E YOSEMITE AVE, MANTECA 95336 <br />Owner/Operator: QUIROZ, OSCAR Telephone: (209) 298-0317 <br />Program Element: 1600 - FOOD PROGRAM Complaint #: C00054480 <br />Responsible Party Contact: <br />DESCRIPTION OF COMPLAINT ALLEGATIONS <br />The San Joaquin County Environmental Health Department received the following complaint: <br />COMPLAINANT ALLEGES THAT A CUSTOMER BROUGHT IN THE FACILITY CLAIMING HIS DOG IS A SERVICE <br />ANIMAL AND THE SERVICE IS EMOTIONAL SUPPORT. THE OWNER OF THE RESTAURANT DID NOTHING. <br />VIOLATIONS AND CORRECTIVE ACTIONS <br />Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br />113700. All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodborne illness. All major violations must be corrected immediately. Non-compliance may warrant immediate closure of <br />the food facility <br />OVERALL INSPECTION NOTES AND COMMENTS <br />FOOD ITEM --LOCATION --TEMP ° F -- COMMENTS <br />No Temperature Data Collected <br />NOTES <br />Complaint inspection. <br />As per person in charge, he is aware of the complaint. The customer was sitting at the back table by the back door with her <br />service animal (dog). As per person in charge, customer informed him that she has a medical condition that requires having <br />service animal to help her. <br />I provided the person in charge with letter for service animal Cal Code requirement ( California Retail Food Code, Section <br />113903). <br />During my visit, I did not see any animal on site. <br />The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br />Safety Code.
