DESCRIPTION OF COMPLAINT ALLEGATIONS <br />The San Joaquin County Environmental Health Department received the following complaint: <br />INFESTED WITH ROACHES. THEY ARE IN THE CUSTOMERS FOOD, THE TABLES, THE BAGS, THEY GAVE <br />COMPLAINANT A BOX AND IT HAD ABOUT 30 ROACHES IN IT. COMPLAINANT PICKED UP AN ORDER FOR A <br />CUSTOMER AND THERE WERE ROACHES ON THE CUSTOMERS FOOD. THIS HAS HAPPENED ON MORE THAN <br />ONE OCCASION. THE FACILITY IS ALSO FILTHY. COMPLAINANT CALLED THEIR COMPANY TO LET THEM KNOW. <br />SAN JOAQUIN <br /> Environmental Health Department <br />CO U NT Y <br />Time In: 8:25 am <br />Time Out: 8:38 am <br />COMPLAINT INSPECTION REPORT <br />Name of Facility: PHO LUCKY RESTAURANT Date: 12/08/2021 <br />Address: 8034 N EL DORADO ST, 95210 <br />Owner/Operator: DOAN, DAVID Telephone: (209) 472-9301 <br />Program Element: 1600- FOOD PROGRAM Complaint #: C00054490 <br />Responsible Party Contact: <br />VIOLATIONS AND CORRECTIVE ACTIONS <br />Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br />113700. All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodbome illness. All major violations must be corrected immediately. Non-compliance may warrant immediate closure of <br />the food facility. <br />OVERALL INSPECTION NOTES AND COMMENTS <br />FOOD ITEM -- LOCATION -- TEMP ° F -- COMMENTS <br />No Temperature Data Collected <br />NOTES <br />I received an email today from the operator saying the facility is ready to open. <br />I arrived at the facility and observed extensive cleaning has been done of the facility. <br />All shelves in at the front counter have products removed and shelves have been cleaned. <br />I observed the kitchen area. <br />I observed cleaning done of flooring throughout and under sinks. <br />I observed cleaning done of electrical receptacle under prep table. <br />I did not observe any cockroaches. <br />Ok to re-open facility. <br />The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br />Safety Code.
