David was not <br />there at the time. I left my contact information with the facility personnel. I was <br />given David's email address as contacting him may be easier through email <br />(david.simms@farmautosupply.com). <br />Discussed with Garrett Backus. <br />Sent David Simms an email and scheduled appointment for inspection on <br />Wednesday, 12/16/2020 at 9:00 AM. <br />Received contact information from Garrett Backus for Jeff Valenti, Program <br />Manager at SJC Solid Waste for the County's Used Oil Program. Garrett <br />suggested that I provide this information to David Simms. <br />12/07/2020 Sent email to David Simms letting him know that we will need to reschedule the MURO <br />inspection to a date once the current lock down is lifted. <br />05/05/2021 Sent email to David Simms to schedule inspection HMBP and HW. MURO <br />09/23/2021 Reviewed narrative and emails. Received a response to my email from David MURO <br />Simms on 5-5-2021 where he reported he had time for the inspection on May 11. I <br />had not seen it until today's review. He asked if I can let him know what my <br />inspections were about and what I will be inspecting. <br />Passing this complaint onto Lead Senior Garrett Backus on 9-24-2021. <br />10/21/2021 Site is on inventory as an used oil collection center and will be inspected on a BACKUS <br />routine basis using PR0546782. <br />5105 rpt <br />Page 1 of 1
