Inspection date: October 19, 2009 <br />Inspector: Michele Henry <br />CLLcED <br />NO' V 1 8 2009 <br />ENVIRUNIVIENT WkPl <br />PE,9N11T/SERVISES <br />#20: Labeling of the hazardous waste container has been updated to reflect the most current <br />information. The company who maintains the containers was notified of their error and reminded that <br />such mistakes cannot occur in the future. <br />#27 & #85: The waste manifest was not forwarded to the site by the program manager. A copy of the <br />manifest is attached to this paperwork and a copy was forwarded to the site for their records. <br />#88: The information has been updated to reflect the most current emergency coordinator information <br />and given place next the telephone for emergency situations. <br />inspection date: October 19, 2009 <br />Inspector: Thuy Tran <br />#5: Monitoring plan is being resubmitted with all relevant information for this site. <br />#7: We are investigating these alarms to see what was done or if action was needed. We believe that <br />the January 13 and 22 alarms were from testing of fixed/replaced equipment as was the February 25 <br />alarm. We are unsure about the alarm on March 5, 2009 as it was not noted by the DO and does not <br />appear to have been going on for a substantial amount of time. <br />#22 and #38: A release statement has been attached to this paperwork. The exact causes are believed to <br />be a break in flex line at the UDC, which has been a problem in the past and was believed to have been <br />corrected. Walton Engineering is investigating why there is fuel in the STP Sump. Initial investigation <br />showed that everything appeared to be tight, making it unclear as to why exactly there was fuel in the <br />sump. We anticipate to have a final diagnosis of the problem by November 20, 2009 with an action plan <br />for corrective actions should one be required. <br />#23: There is no DO inspection on file for these two months. It appears that these may have been lost or <br />not done properly. <br />
