SAN JOAQUIN <br /> Environmental Health Department <br />L: • I -COUNTY <br />Greatness growl J, Time In: <br />Time Out: <br /> <br />COMPLAINT INSPECTION REPORT <br />Name of Facility: LODI MUNI SERVICE CENTER Date: 05/15/2021 <br />Address 408 E LOCUST ST, LODI <br />Owner/Operator: Telephone: <br />Program Element: 2546- RELEASE/SPILL RESPONSE (EXCLUDING JOINT TEAM) Complaint #: COBTLSDZ4 <br />Responsible Party Contact: NICK DAY <br />DESCRIPTION OF COMPLAINT ALLEGATIONS <br />The San Joaquin County Environmental Health Department received the following complaint: <br />A 5-GALLON CONTAINER WAS DUMPED ON SIDE OF STREET AND IS LEAKING AN UNKNOWN OIL ONTO THE <br />GROUND. CAL OES CONTROL #:21-2588. <br />OVERALL INSPECTION NOTES AND COMMENTS <br />NOTES <br />MAY 14, 2021: Per CAL OES CONTROL #:21-2588, a 5-gallon container was abandoned on the side of the street at 408 E <br />Locust St and is leaking an unknown oily material onto the ground. Observed the material that had leaked onto the ground to <br />be a black viscous, tar-like material that had already dried and hardened at the time of my arrival. No water ways were <br />impacted. Nick Day (Lodi Public Works) performed cleanup of the abandoned container and spilled material. Observed the <br />spill area to be adequately cleaned. No staining was observed on the ground. Approximately five gallons waste was generated <br />and will be stored at 1331 S Ham Lane pending proper disposal. <br />Please be aware that all hazardous waste shall be disposed of under appropriate hazardous waste manifesting procedure <br />[HSC 25160 (b)(3)]. Make a hazardous waste determination for all waste materials and dispose of them as required, and <br />provide a copy of the waste disposal record to the EHD. <br />The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br />Safety Code.
